The GAPP (Georgia Pediatric Program) is designed to support children and young adults under the age of 21 who require in-home healthcare services. To qualify for the program, the individual must:

● Have Medicaid insurance

● Case Management & Care Coordination: Ensuring seamless communication and planning across your child’s care team.
